The City of Carlyle Parks Department will be holding Soccer Sign Ups at the Carlyle Park Shed.
Ages 4 – 14 (Kindergarten – 8th Grade)
If you can not make the special sign up day, please complete this form and return it with the proper fees to the Carlyle City Hall.
The deadline for mailed registration forms will be June 5th.
