City of Carlyle COVID-19
Vaccination Point of Distribution (POD) Registration
Signups are only available for those 65 or older living within the city limits of Carlyle, Illinois.
You will be required to provide proof of address.
The date of the Carlyle COVID-19 Vaccine POD is yet to be determined. The number of vaccinations given on this date will depend on the number of vaccinations available at the time.
If you are currently on the County Health Department registry, stay on the list. Just because you sign up on this list does not necessarily guarantee vaccination. If you are not currently on the County list, it is recommended you sign up at http://clintoncountyhealth.com/ or 618-594-6622.
Registering for this list is not scheduling your appointment for a COVID-19 vaccine. You will be contacted by phone regarding the date and time of the vaccination.
If you receive a COVID-19 vaccine, you must be able to commit to getting a second dose of the vaccine 28 days after your first dose.
We will be offering registration on the link below, by calling City Hall at 618-594-2468 – Ext #1, or by physically coming to City Hall to sign up. City Hall is open Monday – Friday from 8 a.m. – 4:30 p.m.
